Boca High Band and Guard Database Form

The following form page is not working. If you have tried filling out this form in the past and haven't seen your name appear, please be notified that your information likely was not received. However, you may still send this info to the email addresses listed below. The database may, in time, be phased out of the web site. The upkeep for such a huge task has become a major difficulty. This is the Boca High Band and Guard Alumni Database Form. We are collecting names of any and all Boca High Band and Guard Alumni. All of the information below will be sent to Tony Reimann and Greg Kuzma at bhsbng.alumni@welcome.to and their personal email addresses. This information won't be automatically added to the database web page, but it will be added to the Boca High Band Alumni Database once I receive the information. Also, by filling out this form, you'll be added to our alumni newsletter mailings which are sent out near the beginning of each month (email addresses won't be revealed in these emails). Please fill out as much information as possible.

If you have a picture of yourself that you'd like to add to the alumni database, either e-mail the picture to me at fizban@gate.net or bhsbng.alumni@welcome.to, send it to me through ICQ at my number (6439325), or send me a link to that picture if it happens to be on the internet somewhere.

Required fields have a star in front. If you are a current or potential member, the only fields necessary are name, e-mail, your member status, and the year you started. Current or potential members are also invited to fill out the suggestions, comments, and instruments played or equipment used fields. If you have other information about band that may be important, such as starting band or guard in a year other than your freshman year or information that is more specific than what's listed or isn't listed, please write this information in the comments box.
